National Grid  
eLearning course on assessing incidents to determine their
immediate, underlying and root causes 
 
As
a preferred supplier to National Grid, TNA has produced
an on-line eLearning course on 'Investigating Incidents'.
The program starts by examining the concepts of hazards
and risks and how they are measured. It then looks at
the possible reasons why incidents happen, including unsafe
acts and unsafe conditions.
All incidents will have an 'immediate' cause (eg: employee
tripped over a low wall) as well as one or more 'underlying'
cause (such as 'lack of warning signs or poor supervision)
and possibly a 'root' cause too, usually a management
responsibility.

Learners
are invited to investigate three imaginary scenarios in
which an incident has taken place: an office trip, an
electric shock whilst drilling in a trench and a fall
from a ladder.
The victim, actual witnesses and supervisors may be interviewed
and we hear their answers to selected questions.
Each scenario concludes with a summary of what happened
and why and lessons learned that would prevent a recurrence.
